What are Forms?

Forms in Kayse are collections of tasks organized into sections. They provide a structured way to collect information from clients, such as:

  • Client intake questionnaires
  • Document collection requests
  • Case information gathering
  • Post-settlement surveys

When you assign a form to a case, all the tasks in that form become "case tasks" that the client can complete.

Creating a Form

To create a new form:

  1. Go to Settings → Forms
  2. Click Create Form
  3. Enter a name for your form (e.g., "Personal Injury Intake")
  4. Add an optional description
  5. Click Create
💡 Tip

You can also create forms from spreadsheets if you have existing questionnaires in Excel or Google Sheets.

Organizing Forms with Sections

Forms are organized into sections (called "Task Lists") that group related tasks together. For example:

  • Personal Information — Name, date of birth, contact details
  • Incident Details — Date, location, description of what happened
  • Medical Information — Injuries, treatments, providers
  • Documents — Upload ID, medical records, photos

Adding Sections

  1. Open the form editor
  2. Click Add Section
  3. Enter a section name
  4. Drag sections to reorder them

Adding Tasks to Forms

Once you have sections, add tasks to each one:

  1. Click Add Task in a section
  2. Search for existing tasks or create new ones
  3. Drag tasks to reorder them within sections
  4. Move tasks between sections as needed

Conditional Logic

Make your forms smarter by showing or hiding tasks based on previous answers:

Example

If you ask "Were you hospitalized?" and the client answers "Yes," you can automatically show follow-up tasks like "Hospital name" and "Upload hospital records."

Setting Up Conditions

  1. Click on a task in the form editor
  2. Click Add Condition
  3. Select the task that triggers the condition
  4. Choose the condition (equals, contains, is empty, etc.)
  5. Set the value to match
  6. Save the condition

Conditional logic helps keep forms short and relevant. Clients only see questions that apply to their situation.

Assigning Forms to Cases

There are several ways to assign forms to cases:

Manually

  1. Open a case
  2. Go to the Tasks tab
  3. Click Add Form
  4. Select the form to assign

Automatically via Case Type

Set a default form for a case type, and it will be automatically assigned when new cases of that type are created.

Via Campaigns

When running a campaign, you can attach a form. Clients who respond will have the form assigned to their case.

Sharing Forms with Clients

Once a form is assigned to a case, you can share it with the client:

  1. Open the case
  2. Go to Tasks
  3. Click Share Form Link
  4. Choose to send via SMS or Email
  5. The client receives a secure link to complete the form

Clients can complete forms on any device (phone, tablet, or computer) without needing to create an account.

Tracking Completion

Monitor form completion progress:

On a Case

The case's Tasks tab shows completion percentage and which tasks are still pending.

Form Statistics

View overall completion rates for a form across all cases:

  1. Go to Settings → Forms
  2. Click on a form
  3. View the Stats tab

Form Templates

Save time by using form templates:

  • Preset templates — Pre-built forms for common use cases
  • Custom templates — Save your own forms as templates
  • Duplicate forms — Create copies of existing forms to modify

Creating a Template

  1. Build your form with all sections and tasks
  2. Click Save as Template
  3. Use the template when creating new forms

Best Practices

  • Keep forms focused — Don't try to collect everything in one form
  • Use clear section names — Help clients understand what information you need
  • Add descriptions — Explain why you need certain information
  • Use conditional logic — Hide irrelevant questions
  • Test your forms — Complete the form yourself to check the experience
  • Mobile-friendly — Remember most clients will complete forms on their phone
